CELL PHONE USAGE AGREEMENT

I, ____________________________________________ agree to all of the following guidelines regarding St. John the Baptist School Board cell phones.

• I have received cell phone number _______________________  
   

• I agree that the care and condition of this cell phone is my responsibility.
   

• I agree to repair or replace the cell phone in the event it is lost or damaged.
   

• I agree, when applicable, to use the two-way communication first.
   

• I agree that if I go over the monthly minute allotment to have the overage charge taken directly out of my payroll.
   

• I agree to only use the cell phone for personal calls in an emergency.
   

• I agree to surrender this cell phone back to the School Board if the Superintendent determines that I have not used this phone for its intended purposes.
